The Commonwealth Home Support Programme (CHSP) is an entry level home help program for older people who need some help with daily tasks to live independently at home. Clients must register with My Aged Care to be part of the program. Diversitat Aged Support staff can help you register or you can call My Aged Care directly. You must be over 65 to be eligible for CHSP services.

Diversitat Aged Support follows the Wellbeing and Reablement model of service. This basically promotes wellbeing and individual needs. The focus is on what are the person’s strengths and how to help someone remain living at home.

The principles under pinning the wellbeing and Reablement approach are that:
- People wish to remain autonomous
- People have the potential to improve their capacity
- People’s needs should be viewed in an holistic way
Reablement involves time-limited interventions that are targeted towards a person’s specific goal or desired outcome to adapt to some functional loss, or regain confidence and capacity to resume activities. Like wellness, reablement aims to assist people to reach their goals and maximise their independence and autonomy.
A person’s needs are best met when there are strong partnerships and collaborative working relationships between the person, their carers and family, support workers, and between service providers.
This health promoting philosophy of care will require service changes, so that clients are supported to gain the greatest level of independence they can and be actively involved in making decisions about their life.
Our activity workers are trained in Easy Moves for Active Ageing (EMAA) and daily gentle exercise is part of programming.
Put simply, the theme is ‘doing with,’ not ‘doing for’ people, and encouraging older people to reach their full potential.

Diversitat Aged Support began a journey of working with clients to produce life stories in different mediums in 2009, when it was explored as part of a “Well for Life” initiative. Our workers have been fortunate to have guidance from authors and historians such as Arnold Zable, Meyer Eidelson and Hazel Edwards.
These meaningful life stories have helped clients, staff and families to celebrate culture, capture memories, document journeys of hope and tell stories of love.
It is a privilege to work with people, their families and carers, to give people an opportunity to produce an individual story that honours and respects them, gives them choice, celebrates their achievements, and has improved emotional wellbeing. This activity has also enhanced transition to residential care for our clients that have made this move.
